To utilize your Behavioral Health insurance, you must meet the criteria and be diagnosed with a Mental Health Disorder.
Please contact your insurance and complete the following form prior to your appointment. All deductibles and co-payments are due at the time of treatment. If you do not see your insurance company listed above then I would be considered an "Out of Network" provider. This means that you would be responsible for paying the full fee up front at each counseling session. You will be given an invoice to file with your insurance company if you choose. Based on your "Out of Network" coverage, your insurance company will reimburse you a percentage of the fee you paid should you file. Below is an insurance questionaiire you can utilize when contacting your insurance company.
